This manual is prepared in accordance with Section 51 of the Promotion of Access to Information Act 2 of 2000 (PAIA). Its purpose is to give effect to the constitutional right of access to information held by private bodies.

• Patient medical records and radiology reports
• Imaging studies (Mammography, Ultrasound, MRI)
• Biopsy results and diagnostic findings
• Billing, accounts, and medical aid claims

4. How to Request Access

A requester must submit a written request to the Information Officer with sufficient detail to identify the record and proof of identity. Fees may be payable in accordance with PAIA regulations.
